What the U.S. can learn from Aussie health care
SYDNEY, Australia ‚ÄĒ Here‚Äôs a damning statistic: Australia spends 8.7 percent of its GDP on health care and covers everyone, irrespective of their employment status. The U.S., meanwhile, spends 16 percent of its GDP on health care ‚ÄĒ far more than any other industrialized country ‚ÄĒ yet 47 million of its citizens lack health insurance while millions more are underinsured.
Critics of nationalized health care paint systems such as Australia‚Äôs as anything but healthy or caring, with putrid public hospitals that offer little more certainty than a long waiting list. This is a point not lost on Australians, with the topic of hospital waiting lists a perennial hot-button topic at election time.
Meantime, here‚Äôs a view of America from Down Under: The U.S. health care debate is at best bemusing to watch, with all those exasperated Americans working up a head of steam in shout fests called ‚Äútown halls meetings,‚ÄĚ most of which resemble a bar room brawl minus the blood. Yes, Australia‚Äôs universal health care is a long way from Nirvana. Indeed, it underperforms the U.S. in some key respects. Yet, lawmakers of all persuasions tamper with it at their peril because they know that most Australians would rather die than give it up.
The great strength of the Australian system is affordability and access. In the U.S., publicly funded health care is confined to those over 65 and the very poor, with the remainder purchasing private insurance if they can (usually through their employer). Great swathes of low-income Americans fail to visit a physician or obtain needed prescriptions or tests because of the potential cost.
Australia‚Äôs Medicare program offers cradle-to-grave health care for everyone. It covers most or all of the cost for physician consultations, as well as specialists‚Äô fees and X-rays and pathology tests. Treatment in public hospitals is free. However, about 40 percent of Australians choose to purchase private insurance, making them eligible for treatment in more exclusive, privately-run hospitals.
Of course, the U.S. system is not without its strengths ‚ÄĒ most notably, far shorter waiting times for elective surgery. A 2001 survey found that 63 percent of Americans waited less than a month, and only 5 percent waited more than 4 months. By comparison, 23 percent of Australians waited more than 4 months.
The reality is that while overall access in the U.S. is inferior, a great many Americans with private insurance receive timely, quality care. This helps explain why President Barack Obama‚Äôs job approval rating is falling: Health care is the perennial ‚Äúthird rail‚ÄĚ subject, with fear of the unknown trumping the potential benefits of a shake-up ‚ÄĒ particularly during a recession.
So does Australia‚Äôs experience with Medicare contain any lessons for the U.S. debate?
Much American fire and brimstone has been aimed at the proposed ‚Äúpublic option‚ÄĚ ‚ÄĒ essentially a universal Medicare-style scheme to compete with private insurance. Critics say this would destroy the private health insurance market and disrupt millions of Americans who are happy with their plans. The warnings are justified to a point ‚ÄĒ some employers may well decide to stop subsidizing coverage. Obama‚Äôs promise that ‚Äúif you like your plan, you keep it,‚ÄĚ is viewed as likely impossible to police.
Nonetheless, the Australian experience suggests that the extra trimmings of private insurance will always remain attractive to those who can pay. Here, the government actually props up the private system by rebating 30 percent of the cost for those who take it up. Recently, this subsidy was eliminated for higher-income earners to howls of complaint from private insurers.
Yet, says Bob Wells of the University of Sydney‚Äôs Menzies Center for Health Policy, ‚ÄúYou‚Äôll always have people claiming it‚Äôs the end of the world.‚ÄĚ
He continued: ‚ÄúThey were saying one million people would drop out of (private) health insurance and it was just rubbish.‚ÄĚ
Another criticism is that nationalized systems ration health care. Yet this is true whenever a capacity constraint exists ‚ÄĒ and is nothing new in the U.S. where Medicare resources are already badly overstretched.
According to Wells, rationing in Australia occurs informally. ‚ÄúIf a hospital has a 90-year-old who needs a hip-replacement and they‚Äôve also got a 40-year-old who needs one that would get them back into the workforce and being productive ‚Ä¶ the decision is made on the ground, by the providers.‚ÄĚ
The Australian government is actually considering a controversial reform to define the ‚Äúuniversal service entitlement‚ÄĚ available under Medicare. Any procedure above and beyond what was deemed reasonable ‚ÄĒ such as the 90-year-old wanting a hip replacement ‚ÄĒ would need to be paid for privately. The effect would be to make rationing explicit.
Yet the bottom line is that the U.S. system rations health care in an even more fundamental way ‚ÄĒ by denying it altogether to people who are unable to purchase coverage. They may simply lack the money or be disqualified because they are already sick and thus too risky to insure.
As Ian McAuley, of the Center for Policy Development, describes the Australian system: ‚ÄúYes there are waiting lists, but it‚Äôs better to be on a waiting list than to be excluded altogether.‚ÄĚ
